Prominent ears — otoplasty and what to expect

Prominent (protruding) ears are a common aesthetic concern that can also affect self-confidence. In children, teasing by peers can have serious psychological consequences. Otoplasty — surgery to correct prominent ears — is a safe, quick procedure with a permanent result.

What are prominent ears

The normal angle between the ear and the bony part of the head (scalp) is 25–30°. With prominent ears this angle is larger — the ears “stand off” the head and visually project forward. The cause is usually:

  • Underdeveloped antihelix — the characteristic fold of the auricle is missing (the most common cause)
  • Conchal overgrowth — deep cavity behind the external auditory canal
  • A combination of both

These can be inherited anatomic features that cannot be resolved non-surgically in adults.

The right time for otoplasty

In children

The optimal period is between ages 5 and 7:

  • Ear cartilage has formed (up to 90% of adult size by age 5)
  • The child has not yet endured prolonged peer teasing
  • Before starting school — preventing social problems

In adults

Otoplasty can be performed at any age. Many patients come in their 20s, 30s, and later after years of self-consciousness about their appearance. The procedure produces the same result regardless of age.

How the procedure works

Otoplasty is a surgical procedure that corrects the angle and shape of the auricles:

  • Anesthesia — general in children; local or general in adults by choice
  • Duration — 60–180 minutes
  • Technique — a small incision behind the auricle (the scar is hidden in a natural fold). The cartilage is reshaped with sutures that form the antihelix and reduce the angle
  • Going home — same day (day surgery)

Recovery

  • Day of surgery: head bandage for 10 days
  • First follow-up: headband / elastic wrap (another 4–6 weeks)
  • Pain: mild, controlled with standard analgesics (acetaminophen)
  • Swelling: subsides over 2–3 weeks
  • Return to daily activities: 7–10 days
  • Sports, contact activities: after 4–6 weeks
  • Final result: 2–3 months

  • Asymmetry — minimal; corrected with fine sutures
  • Recurrence (return to original state) — very rare with modern technique
  • Temporary ear numbness
